For your first appointment...

We're looking forward to your first appointment with us! Thank you for choosing us to be your dentist in Spartanburg, SC! Here are a few things you'll need to remember:


  • New Patient Forms: once your appointment has been scheduled by a member of our office team, we'll send a text notification with a link for you to complete our online forms. If you prefer to come in and fill the forms out in person 15-20 minutes prior, we're happy to accommodate that as well!

  • A Driver's License: As with any healthcare provider or hospital, we require a copy from any adult patient to verify your identity, protecting you from identity theft or a HIPAA violation. 

  • A Dental Insurance Card: if applicable. Please note that in most circumstances, your health insurance and your dental insurance will not be with the same insurance company.


  • Minors (17 years and younger) must be accompanied by an adult/guardian to their first appointment.

  • Confirm your reserved appointment VIA phone because we know your life is busy: We will text or call you one week before your appointment to confirm whether or not you will be attending and text you a friendly reminder 24 hours before. If we are unable to speak with you, we will leave a message. Kindly call us back (you may also leave a message on our automated system or text us back) to let us know whether or not we can expect you so that we may accommodate other patients who may need the appointment if you are unable to attend. If you have not confirmed VIA phone or text within 24 business hours of your appointment, the appointment will be cancelled to accommodate others needing our care. Please note that if you choose to cancel, another appointment may not be available for 2-6 weeks or more.

Because our appointments are in high demand, we regret that we cannot reschedule patients who fail to attend their first appointment without notice.
